Our Registered Nurses provide personalised nursing support at home for agreed clinical needs. We work with you, your family, healthcare professionals and support network. Clinical support is provided after assessment, with consent, and in line with an individual clinical care plan and the nurse’s qualifications and scope of practice.
What we can support with
- Wound and pressure care, where clinically appropriate.
- Medication management within the nurse’s qualifications and scope.
- Continence and catheter care, and related education.
- Chronic disease monitoring and education (for example diabetes or respiratory conditions).
- Enteral (PEG) feeding management and carer education.
- Clinical monitoring and consent-based escalation to your healthcare team where needed.
